Appointment

1. Please call the office to schedule an appointment: 503-221-1581
2. Click on Forms. Yes, there is a lot of information to provide, but the more information you provide, the better I can help you.  If you are able to complete all of the fields (blanks) please click on submit at the end. Better yet, it would be best to both submit the forms electronically and also to print the completed forms and bring them with you.
3. If you can only complete some of the fields now and wish to complete the remainder later, please fill in as many fields as possible and then click on print. Use your printer to print out the forms. Please remember to bring these forms with you to your first appointment.
4. Click on About Us. Please read this section; it contains pertinent information.
5. Click on Directions to get directions to the free office parking structure, which is connected to the office building.
